In order to have a visit, Sunshine Health requires that our office be assigned as the primary care provider PRIOR to the visit. 

 

Please follow the following steps below to ensure your visit will be covered by Sunshine Health. 

 

1) Login to the Sunshine Member Portal or call Member services 1(866) 796-0530

 

2) Select option to change PCP.

 

3) Select Provider Vanessa Blanco Cabrera, APRN

 

4) Record confirmation number and date the change will be effective.

 

5) Notify our office of confirmation number and effective date.
